Payment Information

Most commercial insurances are accepted at Banton Family Wellness.

We accept: Aetna, BC/BS of Florida, Cigna, Medicare, Medicaid, TriCare Standard, TriCare for Life and Staywell's Healthy Kids.

If your insurance carrier is not listed, call ahead to determine if your plan is accepted.

Always be sure to call your insurance customer service to find out if Banton Family Wellness is in their network.

In addition, acceptable forms of payment include:

Cash, Checks, MasterCard, Visa and Discover.

Unless other arrangements are made in advance with the Business Office, insurance co-payments and deductibles are expected at the time of service. To request arrangements, call 763-4700.
